Accessible Learning Services (ALS) Confidential Intake and Self-Assessment Form

Welcome to Accessible Learning Services! This form is designed to gather information to help us understand your disability-related needs and provide appropriate academic accommodations.

While we appreciate many members of our community may not identify with the word 'disability', yet it is a necessary criterion for registering with our accessibility office to ensure learners receive the support they deserve. Recognizing that individuals perceive their identities in diverse ways, it is crucial to respect and validate these perspectives. Whether through identity-first language, such as 'disabled person', or person-first language that emphasizes the individual before the condition, we are committed to fostering an inclusive environment. By acknowledging these differences, we can further empower all learners to access the resources they need to thrive.

This form is for learners who are currently enrolled at Humber. If you are a Guelph-Humber learner, please email our Client Care Facilitator Team at swac@humber.ca to complete your intake form.

Instructions

  • This form will take approximately 15–30 minutes.
  • Complete it in one session as it cannot be saved partway through.
  • Click the Register button at the top left of the screen to get started. You can download a view-only copy of the intake form to review the questions before completing the online form.
